My Jesus have mercy on the Soul of
MRS. AGNES S. KOZAK
BORN
December 29, 1894
DIED
June 19, 1982
O gentlest heart of Jesus, ever present in the Blessed Sacrament, ever consumed with burning love for the poor captive souls, have mercy on the soul of Thy departed servant. Be not severe in Thy judgment but let some drops of Thy Precious Blood fail upon the devouring flames and do Thou O Merciful Saviour, send Thy Angels to conduct Thy departed servant to a place of re¬freshment, light and peace. Amen.
May the souls of all the faithful departed, through the mercy of God, rest in peace. Amen.
Merciful Jesus grant eternal rest.

We would like to express our most sincere thanks to Father Carl Dekat for his spiritual guidance and inspiration. Our faith is the vital element of our lives that turns times of sorrow and sadness into times of hope and joy.

We would also like to thank all of our families and friends for their kindness and thoughtfulness. We are very grateful for the memorial contributions, flowers, cards, and food, but most of all for the warm relationship we all share.

Again, thank you.
